PAIN MANAGEMENT AND PALLIATIVE CARE

Rochester Home Infusion collaborates with pain and palliative specialists as well as hospice care providers to assist patients in need of end of life care and chronic pain management. We partner with programs such as Mayo Clinic Hospice, Season's Hospice and Heartland Hospice to help you and your loved ones improve your quality of life. We achieve this by collaborating with your provider to alleviate pain symptoms, adjust your medication, and help manage side effects. Our comprehensive care model is designed with compassion to deliver comfort not only for you but also your loved ones.

• Our expert nurses meet with you before you go home to teach you and your family about your pain therapy

• We collaborate with your pain and palliative team to manage your pain at home. We assess your pain level, evaluate your response to therapy, and manage side effects. Often, we help titrate your medication dose for better pain control
• We work and listen closely with your family as they understand your level of comfort
• You or your family member can call our clinicians anytime. We are available 24/7
